Cardiac Effects of Vecuronium and Pancuronium during Halothane Anesthesia


Abstract
It has been shown that vecuronium(Norcuron¢ç, Org NC 45), a pancuronium analogue, has no significant cardiovascular change, in contrast to the other competitive neuromuscular blockers. To confirm this fact a comparative study of cardiac effects of vecuronium with those of pancuronium in halothane-anesthetized patients was done. We selected 24 patients in ASA class 1 for pancuronium group, and 24 for vecuronium group. Without any pre medication they took 02(4L/min)-halothane anesthesia (initially 2.0% for five minutes, then 1.0% for ten minutes) under mechanical ventilation (tidal volume; 10ml/kg, respiratory frequency; 12/min).
After anesthetic equilibrium for these 15 minutes, pancuronium 0.1mg/kg in pancuronium group, vecuronium 0. lmg/kg in vecuronium group was injected intravenously. We measured sBP(systolic BP), dBP(diastolic BP), MAP(mean arterial pressure), HR(heart rate), RPP (rate-pressure product) and TTI(tension time index) by noninvasive method, and STI which was measured by analysis of the integral data(QS2, LVET, PEP, PEP/LVET, 1/ PEP2, S2Q and S2Q/QS2) from the electrocardiogram, phonocardiogram, and carotid sphygmogram, in pre-injection state(control value), and every two minutes for 15 minutes after injection in each group.
All these STI values were given by AVL 970 Myocard Check(autonomic computed STI instrument), and corrected by Weissler regression equation. The results were as follows:
1) Compared with control value, there were no significant changes in sBP, dBP, MAP and TTI in both groups.
2) HR and RPP were significantly increased in pancuronium group(p<(0.05), but not in vecuronium group.
